Please bring the following items to your appointment:

  • Photo ID or driver's license

  • The last two pay stubs or a letter of support

  • Birth certificate for non-residents of Texas

  • * Other items if applicable

If you do not have some of the necessary documents to request, call us at 806.322.3599, so we can help you make arrangements.

If you cannot make it to your appointment please call us at 806-322.3599 to cancel. If you are more than 7 minutes late to your appointment you may be asked to reschedule. Please do not bring children to your appointment as we do not have childcare. We apologize for any inconvenience.  Thank you
