

The Patient Navigator is responsible for facilitating and expediting access to preventative and primary health programs in which Haven Health Clinics participates (i.e. Medicaid); Healthy Texas Women (HTW); Family Planning Program (FPP); Primary Health Care (PHC); and Breast and Cervical Cancer Services (BCCS)). The Patient Navigator is also able to assist with other Texas Benefit applications by request (i.e. Children’s Health Insurance Program (CHIP); Supplemental Nutrition Assistance Program (SNAP); Temporary Assistance for Needy Families (TANF)).
For patient navigation assistance, first identify which region you are in and contact the patient navigator assigned to that region.
