Patient Navigator #2

Patient Navigator #2 serves Dallam, Sherman, Hansford, Ochiltree, Lipscomb, Hartley, and Moore.  It is ideal that the Patient Navigator lives in that geographic area.  The Patient Navigator is responsible for facilitating and expediting access to preventative and primary health programs in which Haven Health Clinics participates (i.e. Medicaid; Healthy Texas Women (HTW); Family Planning Program (FPP); Primary Health Care (PHC); and Breast and Cervical Cancer Services (BCCS)). The Patient Navigator is also able to assist with other Texas Benefit applications by request (i.e. Children’s Health Insurance Program (CHIP); Supplemental Nutrition Assistance Program (SNAP); Temporary Assistance for Needy Families (TANF)).
